I'm not sure if your documentation supports the E&M; these are minor procedures, and typically the E&M is bundled in, whether it's a new problem or not. That would be my first thought.
17000 and 17263 can be reported together with the -59, the additional 17263 would not be reported with the -76; that's a code used to report a repeat procedure, not an additional one at the same setting. For example, if you did an EKG, and then later that day did another one. I'm assuming that the lesion is a separate lesion or separate site, within the same anatomical location.
I'd code 99213-25 (if appropriate)
