Is the 24071 excision tumor soft tissue upper arm greater then 3cm only to be done in a facility? We are a derm office and our providers do this in office. We just received a denial from insurance stating.
"place of service 11 has been identified by CMS as a not eligible service preformed in a non-facility setting."
We called the plan hoping they messed up but they confirmed. I am not finding anything in coding books or LCD's that confirms this.
Any help is appreciated. :)
