Ive bn hired to clean the A/R for a stand alone ASC Pain Mgmt. I see the billing staff hs billed a 62321 & 77003 w/DX.M54.16 on a UB. From my research this coding is incorrect yet I'm new to Pain Mgmt. Also facility is charging $12k for 62321. All PPO payors. All insights are welcomed. TY!

Do not report 62321 with 77003, 77012, or 76942.

ALSO, per Novitas LCD Article A56681, M54.16 is an appropriate dx to use with 62321.
