Physician preformed 64622 L3/4,L4/5 and L5/S1 bilateral. How would you bill?

When performing nerve destructions, you count the actual number of nerves the physician injects. One level = one nerve.

L3-64622
L4-64623
L5-64623
S1-64623

64622-50
64623 x 3-50 (or RT/LT...depending on carrier)

(I'm assuming that all levels were performed bilaterally)
