Wiki Billing 99211 w/ pregnancy test

We are having a discussion in our office regarding billing nurse visits and we are wondering if it is correct to bill a 99211 with a 81025? :confused:
 
you can use a 99211 only after the physician has seen the patient in a previous visit for the same dx and has a plan of care in the note for the nurse to follow. A nurse by licensure may not be the primary provider of a service and deliver the diagnosis to the patient. I can think of no scenario where a 99211 could be used for a pregnancy test.

What if the provider see's the patient and has the plan and the patient needs to come back for a pregnancy test to get on accutane because they missed their window, same Dx? Can we bill a 99211 for administering the pregnancy test?

The other scenario would be if they need to come back for their pregnancy test and if negative the provider then issues the Rx. My question for this scenario is... Do we need to bill a 99212 since their is an Rx, or can we bill the 99211 and still prescribe the Rx, or does that look bad if we bill a nurse visit and an Rx is written?
