Wiki Billing 99214 with 95 modifier being rejected

Am I able to bill 99214 with a 95 modifier or does it have to be a lower E/M visit such as 99213?
 
What is the rejection reason? Perhaps it's in a global period. Perhaps the ICD10s are excludes edits. Perhaps the carrier wants other than -95 for telehealth. Perhaps the carrier wants a different POS than you billed for telehealth. Perhaps the documentation does not support 99214. Perhaps the carrier no longer covers telehealth at all.
 
I bill these all of the time. The denial reason, what is it? I know BCBS likes POS 2. I bill 99214 all of the time with modifier 95. Please tell us the denial reason.
