When we bill 99215, 99417 and 90868 with Cigna, they will always deny 99417, EOB says "no additional allowance for an office visit related to a surgery or procedures". We use modifier 25 for 99215 but no modifier for 99417. Is the denial because 99417 needs a modifier?
