Can a nurse practitioner bill a 99215 as long as the documentation supports that code selection? Are there certain insurances that will deny a 99215 from a nurse practitioner?
 
Yes, they can bill this - the coding and documentation guidelines are no different for nurse practitioners than they for any other provider. I have heard that some insurance companies do not allow this code for mid-level providers, and if so, this should be specified in their contracts and/or their reimbursement policies. In my own opinion, I do not think that such a policy is justified.
