Should the billing address be the physicians home address so the checks and EOB's go to his home so if he changes locations then the checks do not stop coming or get lost in the moving cycle? This would be for a physician moving out on his own so we have to credential him at his new location.

I would never send checks to the home address. You can get a PO BOX untill the new address is update. Send in at W-9 and the address can allways be changed.
 
the address in field 32 must be the street address of the office. If the provider wants the checks to go to a different address, home, PO box, billing company, then you have the vendor create a "pay to" field.
