Our practice is new to CCM. Curious if anyone can help clarify somethings for me.
-Can G0506 be added to patients last previous office visit?
-when billing 99490 with 99453 and 99454, are there any modifiers that should be appended?
-Patient enrolls 6/17/20 and has 20 minutes or more time documented by the end of 6/30/20, do you bill all of your patients in July for 06/01/20 or the day they actually signed enrollment papers?

Any and all help is appreciated.
