How do billedfor compound drugs administered in a physician office? They have no NDC numbers. Is J3490 appropriate. If multiple agents are in the same bag, should all of them be referrenced in the comment field of the claim?

What NDC # would you use for 5010?

Are you able to provide an example of the "drug invoice"?
We're new to this kind of billing and I don't understand if the insurance wants to know the ingredients or if they want actual pricing.
