Had a rejection today, as we billed to Humana two doctors who performed the same procedure on different sites on the same date of service. It was legit billing as the doctors performed the procedure on different sites. One doc did the E/M in addition to the procedure. Any suggestions as to what modifiers I should use to get this processed?
 
many question... What were the procedures? why were there 2 different docs? are the docs in the same practice? what did the rejection state as the reason for denial? what was the dx code(s)?
 
I don't have the claims with me now, but we billed two separate claims showing each doc had performed the surgery. It was on a Mohs surgery. One doc did an E/M with a Mohs surgery on both cheeks, the second did a Mohs surgery on the forehead with a biopsy. Both also repaired the sites. It was done in two distinctly different offices of the same building under the same Tax ID. It was denied as a duplicate
