Wiki billing EM level based on addendum

My medical director is checking specialists billing/documentation and is insisiting that the physician can bill a higher code based on an addendum written a few weeks later (not days). this addendum will be lab/radiology results. In one case, the addendum was a call from patient asking for a referral to another specialist. Does anyone know of written documentation that says we can/cannot do this?
