If a patient comes to clinic just for an eye injection (Eylea,Lucentis,Avastin,Ozurdex,etc), when is it permissible to bill also for an E/M and testing (OCT, Fundus photo)?

It is permissible to bill an Evaluation and Management (E/M) code only when additional services, such as those requiring a separate eye exam, are provided. The documentation must be independent and not related to the eye injection.
