We are having issues with Medicare paying for a 93458 when the physician also puts a stent in the same session. We are doing billing for a ASC. I have called Medicare and was told to bill with a TC modifier, I did and claim denied still. We have also reviewed the LCD and one place says that 93458 and C9600 can be billed together, another place says they can't. Can anyone help me as to how to bill these 2 codes in order to get payment for both?
