Can anyone tell me if an internal medicine doctor can bill for making adjustments to Coumadin over the phone?
 
No there is no code to bill for this it should be included in the MDM for the office encounter when the provider sees the patient for a coumadin encounter (99363/99364)
 
Try looking to see if your service/documentation meets the guidelines for CPT 98966-Telephone assessment and managment service. I'm not sure maney ins. companies will consider this a covered benefit though....
 
There is good information in the sectional guidelines in the CPT book. Look up "non-face to face services" and read the sectional. If I remember correctly, these services are included in the office visit within a certain period of time. It'll tell you.

Correct and the call must be initiated by the patient, which this is not.
