Hello,
I work for a family practice, and we are starting to bill Medicare for Chelation Therapy. I found some HCPCS codes (J0470, J0600, J0895, J3520) for it. But should I bill the J code by itself or bill the J code in addition to the IV code (96365)?
Also, I heard to put description such as units or price on box 19 on the claim. Does anyone know more about what to put?
If anyone has experience with this, please advise. Thank you!
 
In my experience, you bill them separately. I was looking into this again last week and I came across a site which gave excellent description for chelation therapy for Medicare:
It is always a good idea to include box 19 for these claim codes to show the units or they will come back requesting them. It states that if a covered diagnosis is not on the claim, the editor/payor for Medicare will automatically deny the claim. Take a look at the site listed above.
