I have read guidance from several different MACs on splitting this billing between 2 claims, using different amounts, and applying either the modifier 59 or 76 (depending on the MAC) to the second claim and adding a comment on both, whether it is the 1st of 2 or 2nd of 2. I cannot however find anything on CMS' Claim Processing Manual that addresses this. If anyone has has success with billing in this instance, would love any additional insight you can provide.
