Is hydration, prior to and following a CT with contrast, billable or integral to the radioogy procedure?

Also, where might we find the "rule" regarding IV hydration prior to and subsequent to a CT with contrast?

I'm most familiar with out patient infusions, chemo, home infusion, ect., but generally Hydration is only covered separately IF for "dehydration" as a separate and identifiable service.
