Has anyone had any carrier denials when billing for both a and b test?
87804 & 87804-59. Billing from a primary care office.
Thanks for your assistance.
 
We are also using modifier QW on both to indicate a CLIA Waived office. Most carriers will not pay for tests outside of a lab unless the office has a CLIA certificate.
