Does the physician or mid level have to be in the office suite when a patient is receiving an infusion?

Yes. Under Medicare rules, ALL services (with the exception of a few things such as vaccinations) must by law meet the definition of a service 'incident to' a physician or NPP's plan of care in order to be covered. In an office setting, the physician or NPP must be present in the office in order to meet this requirement.
