Wiki Billing for two procedures on the same day

Hello Everyone!

I am having trouble billing for Total abdominal hyst(58150) and explatory lap (49000)
the doctor also saw the patient for an initial hospital visit (99222)
how would I bill for this without them bundling everything?

you cannot bill for the xlap once you perform the hysterectomy, the xlap is view as the approach and is inclusive. Also if the surgery was preplanned then you cannot bill the 99222.
