Wiki billing inpatient

Hi,
Please help me better understand.
what is the hours rule when billing inpatient?

examples:
Q#1: if patient admitted on 7/7/14 11pm and discharge on 7/8/14 10 pm, would this be bill as two DOS or one? how would billing look like?

Q#2: if patient admitted on 7/7/14 11 am and discharge on 7/8/14 10 pm, how would this bill, as two DOS or one DOS?

There may be others who do differently, but one of the hospitals my doctors go to, we would bill that as a short stay/observation (99218-99220).

do you bill a discharge in addition to the observation? I been getting denials for observation and discharge. Thanks
