Wiki Billing Lipid Profile/ Abnormal Preventive Visit

We do lipid/cholesterol screenings as part of our preventive visits. I'm getting denials from Moda with the preventive visit turns out to be abnormal. Say, dx z00.121 is billed also with Z13.6, but the screenings themselves were not abnormal. Anyone have any advice? Moda is the only insurance denying these.

Are the lipid lab being done due to dx E78 or E87 Lipids or I50 cardiac conditions or R73.9 Hyperkalemic or HTN II0 problems? What is the ongoing chronic condition of the patient in order for provider ordering these labs? Or go back to the provider by inquiring if any are these supporting problems? Use those above Z codes last on the claim.
