billing of hybrid lenses for keratoconus

does anyone know the proper way to bill for a hybrid contact lense for keratoconus?
 
CPT code 92072 is for keratoconus contact lens fit (we bill it using RT and/or LT modifier(s), according to whatever is done)

We use CPT code 92313 for scleral contact lens fit (ie, Jupiter lens, etc.) again RT and/or LT, modiifer(s).

CPT code 92071 is for "bandage" contact lens, ie corneal abrasion, recurrent exposure. RT and/or LT modifier(s).


Because I do not code for the lens supply, so I am unable to offer any suggestion here.
