HOw would you bill for an office visit if the patient left in the middle of it saying they did not have time. Is there a modifier to use or do I audit visit based on the provider's history and exam?

Wiki billing office visit but pt left early

The visit can only be evaluated on the basis of what is documented as performed.

But an office visit could be billed if it was performed by documentation

Yes that is what I stated. Only what is documented as being performed not what was intended to be done. You only need two out of the three components if this was an established patient. A new patient requires all three components.
