I was told I can bill an e/m code along with the physical code when a px is done if the dr addressed other problems. Wanting to get the rules on this but having trouble finding any info on this can anyone be of any help. Thanks in advance.
 
If the pt is scheduled for a physical exam then presents with other medical problems example breast lump, abdominal pain etc. and the doctor addresses these issues at the same appointment he/she must document the reason for the e/m service in a separate note. You would then append the modifier 25 to the e/m code example 99214-25. And of course your dx would be different from the physical code. Your dx code pointers on the claim must match up as well. Hope this helps.
