Wiki billing pre-op clearance performed by hospitalist

Scenario-

patient is in hospital, the hospitalist is asked to do an H&P for pre-op clearance, how is this billed?
is it different if surgery is emergent?

appreciate input, as I work with the hospitalists and doing e/m documentation training, this question was asked, my thinking is why is the PCP not clearing the patient or the surgeon, especially if their is no pre-existing conditions that would need internal med management before and after surgery, possibly being manage by the hospitalist service.
