Wiki billing preventive and E/M on the same date

Can someone give me information on billing for preventive medicine services and E/M services on the same date. I know that you can do this but it is such a patient dissatisfier and some payors are denying one or the other visit. 1) Do you require the provider to have two seprate documentationa? 2) Do you have the provider explain to the patient at the time that they will be billing two visits? Any information would be appreciated. Thanks!

our practice does bill for the preventative and the OV resulting from services above and beyond the preventative exam. Keep in mind that you can document this on one note but the note must contain some type of verbage "I then turned my attention to address (insert additional medical issues)". This is not a real pleaser with the patients but we always explained to them the additional issues which were addressed. Some carriers will pay and some will not you need to know which carriers in your jurisdiction will & wont for your benefit.
