Hi, how would you bill a radio frequency ablation for T12-L1 level and L1-L2 on the same day. Per LCD guidelines T12-L1 is codded as 64633. Would you use 64635 CPT code for L1-L2 level??? Can you bill 64633 and 64635 together on the same day?
