Has anyone encountered Blue Cross Blue Shield policies with limitations on CPT code 36478? Specifically, I've come across a policy that restricts this procedure to a maximum of four per lifetime, which is problematic for patients needing multiple procedures per leg.

If you've experienced similar issues, were you able to identify any alternative billing strategies or workarounds? Any insights you could share would be greatly appreciated.
