Billing Rules in Virginia

I have a couple of questions and I am looking for some help...

Under Virginia Law:
Can a Nurse Practitioner see new patients?
Can a PA see new patients?
Under the rule for midlevels, when does it apply that the visit needs to be billed at a lower percentage and what is that percentage?
How does it work with ER visits if the patient is seen by a midlevel?

I am getting several different answers to these questions and I am hoping someone can tell me what the law says. Thanks!
