Wiki billing urines on UB04 with what revenue code?

I'm billing urine test (80101/ G0434) to BCBS. They have just told me that I need to now send them on a UB04 with the appropriate revenue code. we are a facility but I am unsure of the right revenue code to use. can anyone help
 
look up the revenue code for either lab or the appropriate revenue code where the test was performed. If you are a facility all of your claims should be a UB04, so I agree with the above question, what kind of facility are you?
