We need clarification on billing Velcade to Medicare. How does Medicare prefer it when administered with a pre-med. They will not accept 96409 as the initial but we think it should.
 
if the velcade is the only chemo given it would be considered the primary (96409), since its usually given as IVP, and any pre-meds would be 96367 (infusion) or 96375 (IVP).

If you're billing correctly I would submit a redetermination with supporting documentation.
