When we have an IUD that comes from the pharmacy, we have typically billed out the J code with a zero charge and the insertion. Would there be an issue with payers paying the insertion if a J code is not billed?
 
No, we bill the insertion only when the IUD is a pharmacy device and have not had any denials with that specific issue.
