Wiki Billing with dx from other clinics records

Is it appropriate to bill for a Depo injection using a dx that was provided in the medical records from another medical provider. This dx may not be one that our clinic has evaluated the pt for. This a student from another location and she had been provided the Depo injections on a continuing basis during her months at the local university. Thank you.

no you cannot use a dx from a different record, you must have a dx documented for each encounter provider by your provider.
