27792 was precerted, and documented in patient chart. OP report reads as bimall with two separate incisions; or could the second fixation be additional ankle support. Pretty sure I'm over analyzing.

27792 is not correct. It is 27814. The report you have above describes bimalleolar ORIF. Open treatment of bimalleolar ankle fracture (eg, lateral and medial malleoli, or lateral and posterior malleoli, or medial and posterior malleoli), includes internal fixation, when performed.
