Can someone please give some input on this. Also any additional information/tips on pacemaker and ICD coding you can give would be greatly appreciated. This procedure hasn't been done yet and is schedule for the 16th of July.

Patient implanted with an 2 port ICD one year ago. He now needs to be upgraded to a BIV ICD which will require removal of the 2 port ICD and replaced with a 3 port ICD. The RA and RV leads will remain, but a 3rd Left Ventricular lead will be hooked to the new ICD.

Would the correct coding for this procedure be 33264 as primary and 33225 as secondary.

Since this claims billing will be for an ambulatory surgery center, only the 33264 will be allowed as CPT 33225 has an N1 Indicator.
