For our Bladder Instillations we use 3 medications that are mixed before placing in patients bladder (HEPARIN,SODIUM BICARB AND LIDOCAINE). When looking at billing portion our financial team has noticed that patients insurance have only been billed for Heparin (J1644). There is another code that is (J3490) Drugs Unclassified Injection that is $0.00. We are unsure if Sodium bicarbonate and Lidocaine are not billable medications and this is how it is supposed to be or if this is something that has been missed and there is a missing code for these medications that we have not been dropping when charting.

Has anyone dealt with this before?
