Hello, Is there a reference in CPT HCPCS or guidelines that defines where the tumor size measurements have to be documented within the chart? Can they be in the heading of the OP report ? Any references would be appreciated .

Hello, There is no reference on documentation of this so as long as it is in OP note it is o.k.
