We need some guidance as this isn't our specialty. We have a patient that our doc charted "near complete blindness, right eye." Looking at the available codes, it is categorizing by level 3, 4, & 5. We don't have any other documentation besides this. How does "near complete" fall into these said categories?
 
It doesn't fall anywhere to be useful to use the proper code. It has tone specified either by visual acuity or visual field. If you attempt to use some level of unspecified code, it most likely will be denied for payment.
