On occasion we have the need to do blood pressure checks without the patient having a regular office visit, or any other services. Our BP checks meet the criteria for billing a 99211 under Trailblazer's guidelines, but I was wondering if anyone might have a form that they use for these types of visits. I would like to use a form to ensure that we have covered all of our documentation bases rather than having my team free form these.
