Blue Shield of CA medical policy

Does anyone have a medical policy prior to 4/5/2013 for ambulatory event monitors and mobile cardiovascular telemetry from Blue Shield of CA. I have rejections from prior dates of service where I know BS of CA was actually paying. Thanks!
