Are your bariatric surgeons documenting BMI on the op report? If no, are you pulling it from the H&P?

Ours document it on the operative note. That way, if the carrier requests the op note for documentation, the BMI is documented.

No, ours do not document BMI on the operative report. I pull from the H&P.

Don't forget that you can pull BMI from nurse notes, flowsheets, anesthesia as well per the I10 guideline
