The procedure for obtaining body mass index (BMI) is only a formula using height and weight. BMI equals a person's weight in kilograms divided by height in meters squared. (BMI=kg/m2). As far as I know there is no CPT that describes this service. However there is an ICD-9 code in the V85 section. I hope this helps.
