If the exam is negative, I know to code the reason for the exam, but, should I use a screen code too? Normally I would just use the reason and send it on. Just want to make sure I am right. Thank you in advance.
 
If the patient was symptomatic and it was ordered as a diagnostic you use the symptoms if the test is negative.. You can use a Z03 code first listed for condition ruled out if supported. If the patient is asymptomatic and the test is ordered as a screening, then the screening Z code is listed first even if there are findings.
