Can someone tell me how to bill for a bone graft to reconstruct mandible surgery code on bill reads 21215 but we also have rev code 278 (implant) with L 8699 which Anthem of Va is rejecting. Should I replace with D7950 and can this code be billed on the UB.

I am new to implant coding and I have the responisbility of correcting the rejecting implant surgery claims, which I am excited about learning
