Wiki Botox Documentation Changes

Does anyone know of any updates from CMS on Botox documentation that take effect in February? I am not exactly familiar with all the details, but apparently Medicare is wanting ongoing documentation of response to Botox injections using approved "scales" or "scores" related to each particular diagnosis. For instance, limb spasticity will now require ongoing documentation of up-to-date modified Ashworth scales to justify continued use of Botox. Dystonia and even migraine will also require providers to document ongoing scores using one of the Medicare approved scales/metrics related to those diagnoses as well.

I have Googled this and I can't find anything? Can someone point me in the right direction, please? Thanks.
