Is anyone familiar with Botox coding and Phenol injection using CPT 64640. My Colo-rectal surgeons want to use it with surgeries (anal) regarding the Botox and in the office for the Phenol for Hemorrhoids?

We have billed 64640 for botox for another MD, some insurances paid. Medicare denied of course.
