Hello!
I work for an independent lab that receives biopsies from physicians all over the US. Lately we’ve been inundated with rejections from Anthem/BCBS to bill our local even though we’re required to bill the state in which the biopsy was taken.
We’re currently billing our information in Box 32. We previously had the referring providers information in Box 32 and our claims were getting paid.
I’ll also mention that we’re billing POS 11, 19 and 22.
Any help would be much appreciated!
